Commit to teamwork with your doctor and care team — it’s one of the best things you can do for your health. Whether you have a clean bill of health or live with any chronic conditions, partnering with your care team can help you be your healthiest.

Here’s how to work with your doctor on your health journey

1. Know your care plan 

  • Ask questions until you understand what to do¹ 

  • Write down your medication names, doses, and when to take them 

  • Know your target blood pressure numbers 

  • Understand what lifestyle changes will help you most² 

2.  Talk openly with your doctor
